We observed that those of our patients treated with rituximab-based regimens who were assigned to the high risk group had a median overall and cause-specific survival of less than four years. When such patients were treated with alkylating agents or with nucleoside analogs median overall survival was also less than four years. This observation indicates that rituximab-based regimens, as well as nucleoside analog/alkylating agent based regimens, may be a suboptimal treatment for such high risk patients. For these patients new treatment approaches are needed. ZAP-70 expression identifies a chronic lymphocytic leukemia subtype with unmutated immunoglobulin genes, inferior clinical outcome, and distinct gene expression profile.
The presence or absence of somatic mutations in the expressed immunoglobulin heavy chain variable regions (IgVH) of chronic lymphocytic leukemia (CLL) cells provides prognostic information. Patients whose leukemic cells express unmutated IgVH regions (Ig-unmutated CLL) often have progressive disease, whereas patients whose leukemic cells express mutated IgVH regions (Ig-mutated CLL) more often ...
متن کاملThe LPL/ADAM29 expression ratio is a novel prognosis indicator in chronic lymphocytic leukemia.
Although the zeta-associated protein of 70 kDa (ZAP-70) is overexpressed in patients with chronic lymphocytic leukemia (CLL) displaying unmutated IGVH genes and poor prognosis, a previous microarray study from our group identified overexpression of LPL and ADAM29 genes among unmutated and mutated CLL, respectively. To assess the prognostic value of these genes, we quantified their expression by...
متن کاملMulticenter study of ZAP-70 expression in patients with B-cell chronic lymphocytic leukemia using an optimized flow cytometry method.
BACKGROUND Flow cytometry allows specific assessment of the expression of ZAP-70, a promising new prognostic factor in B-cell chronic lymphocytic leukemia (B-CLL), but suffers from a lack of multicenter standardization. DESIGN AND METHODS An optimized method for direct detection of ZAP-70 in flow cytometry was tested in a multicenter fashion. Adapted for frozen cells, this method includes a n...
متن کاملBlimp-1 Expression as an Exhaustion Transcription Factor in Chronic Lymphocytic Leukemia
Background: PPreviously, it was shown that exhausted CD4+ and CD8+ T cells in chronic lymphocytic leukemia (CLL) co-express the two immune-inhibitory receptors, Tim-3 and PD-1. Present study investigated the expression of Blimp-1, a transcription factor involved in T cell exhaustion, in patients with CLL. Materials and Methods: Peripheral blood mononuclear cells were collected from 25 untreate...
متن کاملComparison of ZAP-70/Syk mRNA levels with immunoglobulin heavy-chain gene mutation status and disease progression in chronic lymphocytic leukemia.
BACKGROUND AND OBJECTIVES The protein tyrosine kinase ZAP-70 has recently emerged as a major prognostic indicator in chronic lymphocytic leukemia (CLL). ZAP-70 is structurally and functionally homologous to Syk, a key mediator of B-cell receptor signaling. We therefore evaluated ZAP-70 expression in CLL B cells using Syk as an intracellular standard. DESIGN AND METHODS The relative amounts of...
